1. Referral

2. Registration for Diagnostic Services

Location: Diagnostic Services Registration Counter

Process:

Check-In: Upon arrival at the diagnostic services area, check in at the registration counter.
Documentation: Provide the requisition form from your doctor, identification, and any insurance information if applicable..

3. Pre-Test Preparation

Location: Diagnostic Services Area

Process:

Instructions: You will receive specific instructions related to preparation for your tests. This may include fasting, dietary restrictions, or medication adjustments.
Clarification: If you have any questions or need clarification about the preparation, ask the diagnostic staff for guidance.

4. Diagnostic Testing

Location: Diagnostic Rooms / Labs

Process:

You are being directed to CPGRAM website. You would be asked to Register with CPGRAM if not already registered. Registered users can login directly using their CPGRAM user ID and password.

  • 1. Login into CPGRAM
  • 2. In section "Grievance Detail" select "Others/Not Listed/Not Known"
  • 3. In the "Ministry/Department" Drop Down Select ESIC
  • 4. In "Subordinate Department/Office" select the Regional Office
  • 5. Provide details of the complaint
